You can use our secure on-line account as an easy means of access to your electronic medical record. Our Medical Record Portal provides you with direct access via our web-based software. You or your legal guardian have access any time of day, seven days per week.
You can view your appointment schedule, clinical note summaries, your medical profile, medications, and lab results, insurance information and billing balances, and educational resources.

After you've registered we validate the information submitted. When we complete validation you will receive an email that assigns you a user ID and password so you can log in to the portal. The assigned user ID cannot be changed until after you have logged in the first time.
